- Payment for services is required before testing.
- No refunds will be given; payment is final.
- Insurance companies, including Medicaid and Medicare, will not be billed for services provided without a doctor's order.

Notes
- “Panels” are common groups of tests.
- Unless otherwise noted, there is a $12 draw fee and/or $15 swab collection fee on all screening tests.
- These screening tests do not require a physician's order.

RESULTS
- No results will be given verbally at the lab or over the phone.
- Infectious disease results must be given in person.
- Lab results will be available via our patient web portal as soon as they are complete.
- Express Lab is not responsible for confidentially of lab results once they are sent.
- Results may take up to three weeks, please do not call prior.
- You are responsible to consult with a physician for interpretation and care if results are abnormal.
LEGAL NOTIFICATION: Must be 18 or older, be legally emancipated, or have legal guardian present.
Lab staff cannot diagnose or treat patients.
Results are for SCREENING PURPOSES ONLY. You are responsible to consult with a physician for interpretation and care if results are abnormal. If a patient does not have a healthcare provider, one will be recommended.

- The following tests require 12 hours of fasting (No food or drink except water) prior to testing.
