Processes

Your patients are welcome to walk into any of our locations with the form filled out and signed by a provider to have blood drawn and tested. If you draw the patient at your facility, send both the completed form and the specimen with our couriers.

You can also print a laboratory requisition sheet from your EMR if you prefer. We can provide you with pre-printed pads of these order forms upon request.
